Coordinated Veterans' Care (CVC) is a DVA program that helps you manage chronic health conditions with your GP and a care coordinator, usually a practice nurse, to keep you well and out of hospital. You need a Gold Card and one or more chronic conditions, or a White Card and a chronic mental health condition DVA has accepted.
How CVC works
With your consent, your usual GP meets you face to face to write a care plan that sets your health goals and how your care will be managed. Your care coordinator then organises your care under your GP's supervision, with regular face-to-face contact and some phone calls. Your care team can include specialists, pharmacists, physiotherapists and mental health professionals. Care runs in 90-day periods, and you see your GP in person before each new period to review the plan.
Who can join CVC
You may be eligible if you have a Gold Card and one or more chronic health conditions, or a White Card and one or more chronic mental health conditions DVA has accepted. You also need to meet your GP's assessment of who would benefit, live in Australia, and agree to a care plan.
You can't join if you're in the Transition Care Program, living in residential care other than for respite, or have a condition likely to be terminal within 12 months.

Ask your GP whether CVC would help you. CVC stays in place for as long as you benefit from it.
